Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

Test Results

You will be contacted with results after all tests are received and reviewed by the physician. This usually takes 7-10 days after the completion of all tests.

Medication Refills

To refill your prescription, please contact your pharmacy. Your pharmacist will contact our office. Please allow 24 hours to process your refill request. We do not fill narcotic prescriptions after office hours or on weekends.

As designated providers for these carriers, we will file insurance. If your insurance requires a referral for you to see a specialist, please make sure this is in place prior to your appointment.
